In order to provide a process for citizens to register concerns within their communities, the Montana Department of Revenue has created a Citizen Concern Form. This form is used when a citizen has concerns over a potential liquor law violation related to over-service to an intoxicated person, sale to an underage person, disorderly licensed premises, sales/service/consumption on a licensed premises between 2:00 a.m. and 8:00 a.m., or any other worries.
On these forms, the person reporting will be required to fill in the name of the licensed establishment, the address of the establishment, the city, and if possible, the name of the individual of concern, whether it be a bartender, server or seller, owner or bouncer.
When filling out the form, the concerned citizen will also be asked to give a detailed description of the violating events, including dates, times, names, activity of concern, frequency of offending events and any other relevant information. They will also be asked to fill in their name, email address, phone number, and mailing address. The option to report anonymously is available, but it can negatively impact the ability to effectively investigate and address your concern and could result in no action regarding your concern.
Lastly, the reporting citizen will be asked if they are a customer of the licensed establishment, connected to the alcohol industry in any way, or are a concerned community member.
